116-8-第八节 临床中应用糖皮质激素类药物应该特别注意的一些问题
糖皮质激素的临床应用仍处于不断的进展之中,希望有更多的研究能够阐明存在的问题,以下是临床医生用药的一些原则性:

* {1 a. _8 {# f6 G: b: f" l (1)区别高低剂量的差别是非常重要的。有些患者可能的确需要50一80mg的泼尼松,但当给药时间超过2--3个月时,他们肯定会显示出毒性。高于7.5mg剂量的泼尼松对绝大多数类风湿性关节炎患者在任何时候几乎都是禁止使用的,除非患者合并有血管炎。然而,每日1一5mg的泼尼松治疗对许多类风湿性关节炎患者是有明显好处的。 2 S- q2 B5 w. L/ z: J
* `8 {# c5 E( P; S" ~) ~ (2)大剂量糖皮质激素(泼尼松60一80mg/d)可以挽救急性暴发性系统性红斑狼疮、多肌炎和血管炎等疾病患者的生命。但是,剂量必须在6-10周内降至30mg/d或更低,然后用15mg/d或更低的剂量维持治疗。大多数这些疾病患者还必须在开始治疗时采用其他免疫调节剂进行治疗。/ P! A, c" J/ o0 G: T
4 M0 [' y. ~( p, \/ k& E& S. m (3)在美国,75%的类风湿性关节炎患者采用低剂量糖皮质激素治疗。而根据炎性风湿性疾病患者的HPA轴变化的数据,证明这种治疗方案是合理的。虽然仍然有研究人员一直反对在治疗类风湿关节炎时使用任何糖皮质激素类药物,但这一治疗对临床的好转有帮助是不容否认的。

(4)糖皮质激素是罕见的惟一能有效地控制所有严重的慢性炎性风湿性疾病的药物。其中包括类风湿关节炎、系统性红斑狼疮、多发性肌炎及血管炎等。为取得更好的疗效,通常联用其他免疫调节剂。甲氨蝶吟是类风湿关节炎和多发性肌炎较好的治疗药物,而环磷酞胺是治疗血管炎和一些狼疮性肾炎或脑病的较好药物。 ; G b3 Q \* z- N9 f
) B- G$ c' N) ]& i& ? (5)一旦疾病已经得到控制,高剂量的泼尼松必须降低至15mg/d以下。患者接受泼尼松治疗的剂量如果高于30mg/d,剂量可以以5-10mg/周逐渐降低。当剂量低至20mg/d时,剂量应以2.5.5mg/(2-4周)的速度降低;当剂量降到10mg或以下时,爱.康.之.家.并以lmg明 的速度逐渐撤出。 8 }$ R8 U0 L$ [

(6)糖皮质激素的副作用与剂量及治疗的持续时间有很大关系。当患者需要长时间的尤其是高剂量糖皮质激素治疗时,必须对骨质疏松症进行干预治疗。治疗方法依据测得的骨密度而定。对即将接受长期低剂量糖皮质激素治疗的患者,应定期测量其骨密度以防止出现骨质疏松。

& B9 f f! ?0 X1 J; K) u# Y# O% M (7)不少风湿病学专家采用长期持续低剂量糖皮质激素治疗炎性风湿性疾病,而不是采用间断治疗。因为许多风湿病学家和患者都经历过如下情况:在取得满意治疗效果后停药,而在停药1-6个月后暴发性复发。普遍认为,不固定低剂量(1一5mg/d)的糖皮质激素治疗对许多患者有明显的疗效。但仍然需要长时间大量的临床研究对此予以证实。普遍应用的随机、双盲、对照试验对糖皮质激素的临床研究有着许多明显的局限,尤其是较短的时间限制。另外,这种试验也不大可能提供对风湿性疾病糖皮质激素最佳治疗方法的最准确的信息。临床医生可以通过自我报告调查表来监测患者长期的数据,并从中获得糖皮质激素在风湿性疾病患者中的最佳应用方法。
