12-7-第七节 T细胞在RA研究中的展望
单纯的认为RA是由T细胞介导的或RA是由非T细胞引起的疾病都是狭隘的。人为分隔不同种类的细胞和细胞因子在RA发生中的作用是不明智的。因为在RA患者关节滑膜组织的微环境中,不同种类的细胞和细胞因子发生着极其密切的接触和反馈调节。可能多种途径,包括由“关节抗原”引起的特异T细胞免疫反应和不依赖抗原的非特异免疫反应均参加RA的发生。它们并不互相排斥,不同的机制可能在疾病的不同阶段分别发挥作用。CD4+T细胞特异的识别“关节抗原”可能在疾病早期的起始方面起关键作用。随后巨噬细胞的活化及其分泌的两种重要的炎性因子TNF-α和IL-1,使疾病的发展得以维持。在疾病晚期,滑膜成纤维样细胞不依赖于T细胞的自主增生以及受 IL-1刺激分泌的多种金属蛋白酶,是造成软骨和骨破坏的主要因素。在这个阶段,T细胞起的作用可能很少。

展望RA的研究,阐明HLA-DR4基因是如何影响RA的发生,可以帮助理解T淋巴细胞在RA发病中的作用。RA研究的主要谜团仍然是对潜在“关节抗原”的发现和抗原特异T细胞克隆的阐明。患者关节滑膜组织巨噬细胞活化的详细机制则有待进一步的深入研究。T细胞介导的免疫炎症反应是如何引起滑膜成纤维样细胞的自主增生,也是一个有待解决的问题。同时,那些在患者滑膜组织调节免疫炎症反应的新的细胞因子及其作用也有待发现。分子生物学技术突飞猛进的发展,必然使人们在未来的若干年内能够回答这些问题。这些问题的回答不仅对人们了解RA的发生机制具有重要意义,而且也必将对发展新的治疗RA的手段产件几巨大影响。
